This form is required for your first visit. Please carefully read and fill it out.

Please fillout the Client Intake Form and email it to us prior to your first appointment. The more we know about your life and the stresses and traumas of your life the better we can integrate the work and be efficient. Take some time and think about your goals and desire for the session as that is what we will focus on.

Any appointment date changes or cancelations must be made 2 working days in advance of the appointment. You may change the appointments online or by calling the office (403 2501627). Any changes requested with less than 2 days notice must by made by phone.

Appointments changed or cancelled with less than 2 working days notice, may be charged a fee of 40% of the service booked.

Missed appointments will be charged at 50% of the session booked. The fee goes into our charitable fund to assist low-income families that need help.

Last minute appointments can only be made by phone or email. The online calendar requires 24 hours for an appointment. Please feel free to call or email if you have an emergency or need a last minute appoint. We do our best to fit you in.

Our Family Policy

If several members of the family have similar allergies and need the same treatment, we will arrange to do all of them at the same time. You will only be charged one fee for the whole family. If, however, each one needs different treatments, a separate appointment will need to be booked.

What To Bring

If you are booking for allergies or headaches, please have handy, samples of anything you suspect you are reacting to. Just a drop or two in a baggie is sufficient but be sure it is labelled please.

Everyone, please have on hand any medications, supplements, and nutritional products you take. These are tested to be sure you aren’t allergic to anything and to determine whether your body is absorbing and using them properly.

What About Small Children?

If you are booking a child for the appointment, please be prepared to hold the child and participate in the treatment yourself. Choose a time when the child is likely to be sleepy. For Toddlers we work the same way but bring some books and toys to keep him or her occupied. Snacks are a good idea too but I will allergy test them first to be sure there is no reaction that would interfere with the work.

If the appointment is for you, any children will need someone to care for them unless they are old enough to sit and read or entertain themselves the whole time without interrupting you. We do not provide childcare during the appointments.

How Often Will I Need To Come?

That depends on what you are seeking to achieve and how balanced or unbalanced your meridian system is. After the first appointment we can work out a plan and give you a better idea. Generally acute issues such as a broken bone, concussion, burns, severe allergic reactions will need several session close together to get thing under control. For more chronic issues usually once a week or once every two weeks is best.
